Cancelation Policy

Please respect a 24-hour cancelation policy or expect to be charged for $75 for your session. I accept cash, check, or credit cards including HSA. Some scholarships and sliding-scale rates may be available upon special request and circumstances. Otherwise my fees are as posted with the package and session rates. Please note that while tips are appreciated, they are not expected. I charge what is fair for my experience, education, work, and time. Insurance/Receipts: I do not bill any insurance companies. I will provide you with a receipt for your payment if requested. Service code and reason for service codes and information are provided as a convenience to both client and carrier and are not intended to be construed as a diagnosis or treatment of any symptoms. If you require a diagnosis, that must come from one of your medical providers. Each 75-minute session will customarily include time that is not hands-on work, such as check in, consultation, visual assessment, education pieces & exercises, and checkout. Thus, I have one standard receipt form that is coded for a 60-minute minimum amount of hands-on table time. I do not customize forms to meet individual requests or slight variations. If copies of any additional office records are required, they will be provided upon a release of information form. A service fee may apply for records requests as permitted under state law.
